        <jats:abstract>
          <jats:p>İn this study, evaluation of antibiotic usage in the community of Konya district was aimed. 206 patients, who used antibiotics during the last month were included in the study. A questionnaire form was applied face to face to the patients. 18 % of the patients did not have a health assurance. İt was found that antibiotics were proposed to 91.3 % of the patients by doctors. İt was determined that about 3/4 of the patients used antibiotics for respiratory infections. Radiological examinations were the most requested examination and amoxicilline-klavulanat was the most frequently used antibiotic by the patients. 44.7 % of the patients declared feeling healthy as the reason of quitting the antibiotic use. There was no significant relation betvveen antibiotic usage properties and educational level of the community. İt was concluded that, the doctors should also be educated with the community about acceptable antibiotic usage.</jats:p>

        <jats:abstract>
          <jats:p>The effects of surgery and anaesthesia on endocrine functions are well known. Hovvever detailed studies about the effects of propofol on thyroid functions are absent. The aim of this study is to study the effect of propofol anaesthesia on thyroid hormones and to compare it with isoflurane anaesthesia. This study was performed on ASA l-ll class.euthyroid 30 patients vvithout endocrine pathologies. After premedication, induction was performed in the first group (n=15) with i.v. 0,05 mg fentanyl, 5-7 mg thiopental; in the second group (n=15) with i.v. 0,05 fentanyl, 2 mg/kgpropofol. Patients were intubated with i.v. 1 mg/kg succinylcholine, maintenance was done with %1-1,5 isoflurane %50 O2-%50 N2O + atracurium in the first group and 6-10 mg/kg/h propofol + %50 O2+%50 N2O atracurium in the second group. Blood samples were obtained before induction(control), after induction, first hour intraoperatively on postoperative 2nd and 24 th hours chemiluminesance method was used for the plasma TSH,TT3, FT3,İ7'4, FT4 analyses. The TSH serum levels did not change in both groups. A significant decrease in TT3, FT3 serum levels in the 24th hour postoperatively was observed in the first group (p&lt;0,01), (p&lt;0,05).This decrease, in the first group was significant betvveen the groups (p&lt;0,05). Significant increases in serum TT4 levels occured first hour intraoperatively in the first group (p&lt;0,05). No difference was found betvveen the groups. Asignificant increases in serum FT4 levels was recorded in the 1st hour intraoperatively and 2nd hour postoperatively in the first group(p&lt;0.01) (p&lt;0.05)The increases was significant betvveen the groups (p&lt;0.01)(p&lt;0.05). We conclude that propofol is saf e alternative agent to isoflurane in hyperthyroid patients.</jats:p>

          <jats:p>Aim: Free radical induced lipid peroxidation has been suggested as a possible pathogenic factor of preeclampsia. İn this study, we were aimed to measure antioxidant enzyme activity in erythrocytes in maternal and fetal circulation in preeclamptic patients in the third trimester. Material and metod: Maternal and umblical venous blood were obtained from thirty preeclamptic and thirty-one normotensif women with singletton pregnancy in the third trimester. The activites of glutathione peroxidase (GSH-Px), catalase (CAT) and superoxide dismutase (SOD) were determined in plasma and erythrocytes. Results: Patients ages and gestational weeks were not different in both groups (p&gt;0.05). İn maternal erythrocytes of patients with preeclampsia, whilst GSH-Px and SOD levels were significantly higher than control (1857±13.2 vs 1387±123.8 U/gHb and 2593.2+330.7 vs 2041+200.3 U/gHb; p0.05, respectively). Independent t test was used for statistical analysis. p&lt;0.05 was accepted as significantly. Conclusion: The results demonstrate that increase in oxidative stress in preeclampsia results in development of defence mechanism both in maternal and fetal circulation to protect against reactive oxygen species. CAT has no impact on this protective effect in preeclampsia.</jats:p>

          <jats:p>İn this study the effects of low-dose morphine hydrochlorur on rat kidney vvere determined at light microscopical level. A total of 30 Sprague Davvley rats from both sexes (15 male and 15 female) vvere used. Five rats from both sexes vvere served as Controls, vvhereas 10 rats from each sex vvere used as experimental group. Control animals vvere injected subcutaneously vvith physiological şaline (5mg/kg), the animals of experimental group vvere given morphine hydrochlorur at a dose of 5 mg/kg in every other day. At the end of the experiment, the animals vvere perfused vvith buffered formaline (pH=7.4) under anaesthesia, and kidneys vvere extirpated, and morphometrical measurements (thickness, height, length) vvere taken. Follovving, kidney samples vvere processed by means of routine histological techniques and the sections vvere taken at 7pm thickness, and stained. Slides vvere then examined using light microscope. The investigations have revealed that the low-dose-morphine hydrochlorur caused to glomerular destruction, epithelial necrosis and desquamation in both proximal and distal tubuli, vascular hyperemia and hemorrhagies. Based on the findings, it was concluded that the results of this study might be specific to the rat kidney vvhich is devoid of detoxification enzymes in tubuler epithelial cells and a study should be performed on an animal having similar metabolism with human.</jats:p>

          <jats:p>Introduction: İn this study, we presented the results of smooth pinning which is a less aggressive surgical procedure with pelvipedal cast application in fractures of the neck of the femur in children. Materials and Methods: This study covers the results of surgically treated 48 hips of 49 children with fractured neck of femur who were admitted to our clinic betvveen 1986-1998. The mean age was 6.2 years (range 2-9 years). According to Colonna classification, four hips were type I, 27 hips were type II, 16 hips were type III, and two hips were type IV. İn ali patients, closed reduction with three or four percutaneus or öpen surgical smooth pins of two or three millimeters in diameter was applied using image intensifier and traction table. Pelvipedal cast was applied postoperatively. Results: Patients were follovved for 43 months (range 27-163 months) and the results were assessed using Ratliff’s criteria. Thirty-nine (79.6%) hips had good, nine (18.4%) hips had fair, one (2%) hip had poor results. Complications included avascular necrosis in 10 hips, prematüre closure of the epiphyseal plate in 3 hips, non-union in 3 hips, varus deformity in five hips, pin loosening in three hips. Conclusion: We realized that smooth pins were advantageous as their damages on epiphyseal plate were less, their percutaneus applications were easier and removing them by minör surgical procedures was possible. Unlike in adults, the femoral fractures in children are healed easily vvithout using compressive systems. İn relation to this, we observed that smooth pins had no negative effects in fracture union. The complication rates in this study were not higher than other studies reported in the literatüre.</jats:p>

          <jats:p>The aim of this study was to investigate the effect of clarithromycin on the isolated pregnant human myometrium contracted by oxytocin. The myometrium strips (15X3 mm in size) were obtained during caesarean section. Different concentration of drug; 0.5 mg/ml (Group I) and 1 mg/ml (Group II) was used and no drug was applied to the placebo group (Group III). The two different doses of clarithromycin inhibited the amplitudes, the inhibition was dose and time dependent. Hovvever the frequency was increased independent of dose and time. No changes was observed in both amplitude and frequency of the placebo group. İn conclusion, clarithromycin dose dependently inhibits the amplitude of contractions but also increases frequency of contractions induced by oxytocin in human myometrial strips in vitro.</jats:p>

          <jats:p>Objective: The compare the effect of salpingo-oopherectomy and adnexial detorsion on the lungs of the rats which have experimentally tvvisted adnex. Material and Method: 28 wistar female rats randomly selected and divided into four groups. The group 1 is detorsioned and the group 2 has salpingo-oopherectomy 24 hours after adnexial torsion.The other group had laparatomy (group3) and the rest had only anesthesia. Twenthfour hours after the second procedure ali the rats decapitatedand their lungs evaluated microscopically and macroscopically. Findings: The detorsioned group has more pathological microscopic and macroscopic changes meaningfully. Result: Detorsion procedure incrases the pathologic macroscopic and microscopic changes at the lung</jats:p>

          <jats:p>During 1996 and 1999, we applied MVAC in 27 patients whose final stage bladder tumor depends on pathological and radiological bases at urological department. 15.4 % had complete response, 34.6 % had partial clinical response, 34.8 % hadn’t any clinical response and 19.7% had progression. One of the patient died due to a wide spreed tumor metastasis and neutropenic sepsis during the treatment. The most common side-effects were nausea- vomiting, transient renal failure, leucopenia, and trombocytopenia. M-VAC therapy showed it’s beneficial effect by decreasing the stage so providing TUR possibility and other alternative surgical procedurs.</jats:p>

        <jats:abstract>
          <jats:p>Aim: İn this study, the mechanichal resistance of the different kind of anastomoses made traditional suture materials and fibrin glue, and related celi infiltration in small intestine anastomoses performed after mechanical obstuction investigated. Material and Method: This study was performed on 30 rats which were divided into two groups (15 Each) After 24 hours of the mechanical obstructions was made and after 24 hours a traditional suture material anastomoses was performed in one group while fibrin glue anastomose was performed to other group. The bursting pressure of the anastomoses and perianastomose celi infiltration were invastigated in both groups three days later. Results: We observed that, the usage of fibrin glue did not affected the mortality while was found to be more reliable considering leake from the anastomoses . The bursting pressure of the anastomoses made by suture material was 25.7 mmHg which was found to be 23.7 mmHg when fibrin glue was used. Hovvever, there was no statistically significant difference betvveen those two results. Also the celi infiltration was observed to be more when fibrin glue was used compared to other group, but the differce was not statistically significant. İn conclusion, the present study shows that the usage of fibrin glue in accurate concentration and amount might be an effective method in preventing leakage after anastomoses.</jats:p>

        <jats:abstract>
          <jats:p>Postoperative heterotopic ossification (HO) around the hip is common follovving total hip arthroplasty and after surgical treatment of the acetabular fractures. Hovvever, its occuring after intramedullary nailing operations for femoral fractures is relatively uncommon and not well documented. The purpose of this study was to evaluate the clinical importance and aspects of heterotopic ossification after intramedullary nailing. Between October 1995 and March 1999, 70 patients treated by intramedullary femoral locked and reamed femoral nailing were follovved and studied, prospectively. There were 63 male and 7 female and average age was 34 years (17 to 70). To determine of HO, the criteria described by Brumback et al. were used. Mean follow-up was 18 months (9 to 48 months). Ten patients were lost to follovvup, thus 60 patients remained were evaluated. Grade I HO developed in 4 patients (7%), grade II in 1 patient (1.5%) and grade III in 1 patient (1.5%). The patients who developed HO were treated by conservative methods with success. İn conclusion, the findings of this study support that heterotopic ossification after intramedullary femoral nailing is uncommon and not a great problem in management of this patients.</jats:p>

          <jats:p>Increased intraabdominal pressure decreases the venous return due to compression of vena cava inferior. Lower part of body is supposed to be affected mostly from this hemodynamic changes, especially microcirculation and aftervvard oxygen saturation in lower extremities. We investigated the relationship betvveen intraabdominal pressure and oxygen saturation of lovver extremity measured by pulse oxymetry and vvhether oxygen saturation of leg is an indicator for intraabdominal pressure. VVe employed on 5 dogs vveighing mean 14.2 kg. Under general anaesthesia, pneumoperitoneum was established via Veres cannula and normal air insufflation at 6 different pressure levels ranging 0-50 mmHg and 3 different time intervals. Measured lovver extremity oxygen saturations vvere analysed by bivariate correlation. İntraabdominal pressure shovved a significant negative correlation with lovver extremity oxygen saturation(p=0.001, r=0.97). As a conclusion, lovver extremity oxygen saturation seems tobea reliable indicator for increased intraabdominal pressure. Further clinical and experimental studies are necessary in this subject.</jats:p>

        <jats:abstract>
          <jats:p>Everybody wish a less invasive surgical procedure applied at Benign Prostatic Hyperplasia (BPH). We evaluated the results of TUIP application at our clinic to 14 cases with BPH Identification in point of objective measures (Uroflovvmetry), subjective measures (Symptom score) and sexual function. Average age of 14 cases subjected to TUIP was 54.8 and average observation period was 35.5 months. İn ali cases pre and postoperative symptom score (IPSS) and uroflovvmetry data have been evaluated and compared. A reduction in the symptom score has been observed after the TUIP. Maximum urine flovv rate increased from 9.8 ml/sec to 14.3 ml/sec at sixth month. Post-operative retrograde ejaculation has been observed in only one patient. Although recovery rates vvere lovv as compared to TURP, TUIP was evaluated to be an effective method in obstruction therapy at carefully selected patients.</jats:p>
