          <title>Comparison of TC-99M sestamibi with I-131 whole Body Scans in Differentiated Thyroid Carcinoma Patients</title>

        <jats:abstract>
          <jats:p>This study evaluated the potential of Tc-99m sestamibi whole body scan as an alternative to whole body I-131 scan for well differentiated thyroid cancer patients prior to I-131 ablation and post ablation states. We evaluated 115 patients with differentiated thyroid cancer who had total or near total thyroidectomy. All patients had undergone Tc-99m sestamibi whole body scanning high resolution neck ultrasonography prior to conventional I-131 whole body scanning, thyroglobulin and antithyroglobulin values measured. Also all patients had neck and chest CT without contrast agent. The results showed that Tc-99m sestamibi whole body images are highly sensitive and specific in differentiated thyroid carcinoma patients without thyroid supression (T4-off), compared to conventional I-31 whole body scans (kappa=0.68 p&lt;0.001). Neck USG with Tc-99m sestamibi is comparable with conventional iodine based whole body studies also compared to high döşe post-ablation 6th day I-131 whole body scans. Neck USG increases sensitivity and decreases specificity of Tc-99m sestamibi scans. Clinical investigations had to be made to evaluate the efficiency of Tc-99m sestamibi in patients receiving thyroid supression.</jats:p>

        <jats:abstract>
          <jats:p>Aim: To evaluate the efficacy of a single döşe epidural corticosteroid injection in the treatment of lumbosacral radiculopathy secondary to lumbar disc herniation. Methods: We administered single dose epidural injection of steroid and local anestetic mixture in 31 patients with radicular pain due to lumbar disc herniations. Outcome was measured using a visual analog scale ranging from 0 to 10; straight leg raising test (SLR) values; a measurement of finger-to-floor distance; a patient satisfaction scale; and a Roland Morris low back pain questionnaire. Results: Thirty one patients were followed fors ix weeks. We found significant improvement in all parameters in patients at second day, second week and 6th. Week postinjection (p&lt;0.05). Conclusions: We concluded that single-shot epidural corticosteroid injection is effective in the treatment of lumbar radicular pain due to lumbar disc herniations.</jats:p>

          <jats:p>Purpose: To determine the effect of NAC therapy on plasma levels the protective role of tissue injury and organ dysfunction of free oxygen radicals in experimental sepsis model. Material and method: 30 female rabbits were divided into 3 groups, each composed of 10 rabbits. Sepsis was constituted by caeucum ligation perforation tecnique. Groups named sham, sepsis and sepsis + NAC (10mg/kg/day) were formed and medication was given 12 and 24 hours after caeucum ligation and perforation. At the same time blood samples to measure blood gases, plasma glutathion (GSH), malondialdehyde (MDA) and biochemical diagnosis were collected. Tissue samples were obtained. Findings: Plasma GSH levels were elevated in NAC group with respect to sepsis group (P&lt;0.05). Plasma MDA levels were decreased in sepsis group when compared to NAC group (P&lt;0.05). Liver tissue MDA levels were found to be same in both groups. AST, ALT and creatinine values were elevated significantly in NAC group when compared to sepsis group (P&lt;0.05). Apoptosis in liver tissue was significantly decreased in NAC group when compared to sepsis group (P&lt;0.05). Degree of mononuclear infiltration in kidneys were decreased in NAC group when compared to sepsis group (P&lt;0.05). Conclusion: Application of low dose NAC an antioxidative agent in experimental sepsis model has beneficial effects on pulmonary functions and plasma MDA, GSH levels. More and comprehensivestudies which tests different NAC doses are needed to determine the role for NAC in experimental sepsis model.</jats:p>

        <jats:abstract>
          <jats:p>The first CABG surgery cases were performed on the beating heart, because extracorporeal circulation and cross clamping the aorta provided optimal surgical comfort this approach has gained wide acceptance worldwide. However in high risk patients, this technique carries inherent risk that may results in limiting indications for surgery. This Study reports our initial clinical experience on off-pump coronary bypass surgery especially in patients with porcelain aorta or poor ventricular function.</jats:p>

          <jats:p>Nearly all of the reports about pneumomediastinum associated with pneumonia have been conducted in adults as pneumomediastinum was uncommon in pediatric age group. This case report is described an infant with pneumonia complicated with pneumomediastinum and epidural emphysema. Her original disease is a severe combine immunedeficiency (SCID). Another reason to provide this case report is that epidural emphysema or pneumorachis associated with pneumonia and pneumomediastinum is a rare condition.</jats:p>

          <jats:p>The latex allergy has been increasingly recognised in recent years in both adultsand children. After anaphylactic type reactions due to latex has been shown as case reports, latex allergy became an important problem especially in medical practice. The majör risk factors in latex allergy are intense exposure to latex allergens (surgical gloves, catheters, etc.), atopy hand eczema, female gender, history of multiple operations and dental interventions. Some occupational groups such as health-care workers and workers in latex industry are under greater risk due to their possible frequent contact with latex. An association between latex allergy and allergy to various fruits (banana, avocado, pineapple, kiwi, etc) has been reported. Latex allergy is responsible for a wide spectrum of clinical symptoms ranging from a mild dermatitis to severe anaphylaxis in both adults and children. There fore an allergologic investigation for latex allergyshould be considered for those from high-risk groups. Then, it will be possible to take precautions in order to prevent life-threatining allergic reactions by exposure to latex</jats:p>
