          <jats:p>Propolis is a resin collected by honeybees from various plant sources, has attracted the attention of researchers due to its several biological and pharmacological properties in recent years. The aim of this study was to investigate the antimicrobial activity of ethanolic extract of propolis (EEP) collected in Kayseri and its surroundings against some clinically important microorganisms. In the study, Staphylococcus aureus, Enterococcus faecalis, Escherichia coli, Klebsiella pneumoniae, Helicobacter pylori, Candida albicans, Candida glabrata strains were used and agar dilution method was performed to investigate of antimicrobial activity according to the CLSI. By using microbroth kinetic system based on continuous monitoring of changes in the optical density of bacte¬rial growth, turbidimetric growth curves of a model bacteria (S.aureus) in the presence of increasing concentrations of EEP were also generated and determined minimum inhibitory concentration (MIC). MIC values of EEP against tested microorganisms were found between 64 and 1024 µg/ml by agar dilution method. The results in microbroth kinetic system were similar to the agar dilution method for S.aureus and in vitro antimicrobial susceptibility was determined in an earlier stage. Kayseri propolis have shown antimicrobial activity against common pathogens, is important to evaluate the availability of treatment.</jats:p>

          <jats:p>This study was carried out in order to assess the inpatients’ satisfaction levels with physical condition, doctors, nursing, cleaning and food services in Afyon Kocatepe University Ahmet Necdet Sezer Research and Application Hospital. A total of 345 inpatients were participated in this study. A questionnaire was prepared to evaluate the satisfaction levels of inpatients. A face-to-face interview approach was used. The mean satisfaction levels of inpatients were 4.18±0.77 for physical conditions, 4.20±0.90 for doctors, 4.35±0.84 for nursing, 4.14±0.96 for cleaning, and 3.99±1.05 for food services. Statistically significant difference was found between educational status and their satisfaction levels (p</jats:p>

          <title>Demographic Features And Functional Status In Patients With Failed Back Surgery Syndrome</title>

          <jats:p>The aim of this study to evaluate the demographic features in patients with failed back surgery syndrome (FBSS). Material and Fifty eight patients who underwent spine surgical intervention on a variety of reasons that ongoing complaints of postoperative recurrent back, back-legs and/or legs pain that admitted to policlinic were evaluated retrospectively. The patients were assessed in terms of pain by visuel analog scale (VAS), in terms of functional status by Oswestry Disability Index (ODI), in terms of reasons of operation and types of operation by history andin terms of lumbar mobility by Lumbar Schober’s Test (LS). The mean age of the patients were 40.52±8.15. The patients were 61.3% females 38.7% males. 88.1% of the patients had been operated for lumbar disc herniations, 5.3% of the patients had been operated for spinal stenosis and 6.6% of the patients had been operated for osteoarthritis. Discectomy surgery has been performed in 55.3% of the patients, discectomy+laminectomy surgery has been performed in 30.5% of the patients, fusion surgery has been performed in 5% of the patients and laminectomy surgery has been performed in 9.5% of the patients. VAS and ODI values of the patients were 7.20±1.74, 41.2±4.8, respectively. LS was evaluated under 15cm in 93% of the patients. FBSS that affects to be negative about working and daily life with severe pain and limitation of lumbar motion is a significant problem for patients and physicians.</jats:p>

        <jats:abstract>
          <jats:p>Suprascapular notch located on the superior border of the scapula and just medial to the base of the coracoid process. This notch is covered by superior transverse scapular ligament and is turned into a foramen. Suprascapular nerve passes under this ligament and suprascapular vessels passes together over it. The images of the patients undergone MDCT examination between 2008 and 2009 were used in this study. Total 88 right and left MDCT scapula images obtained from 44 individuals (22 male, 22 female) were used. Suprascapular notch was divided into 5 types from these images. These types; Type 1: with deep and large notch, Type 2: with deep and narrow notch, Type 3: with shallow and large notch, Type 4: with shallow and narrow notch, Type 5: without a notch. In our study, the most common type was with (29,54%) deep, large notch and the least common type was the group (2,27%) without a notch. In addition, the relationship between suprascapular notch and supraglenoid tubercule, superior angle and inferior angle was evaluated.</jats:p>

          <jats:p>The aim of the study was to determine the effects of alpha lipoic acid (ALA) on the lipid peroxidation and antioxidant status. Twelve rats were included in the control group. 48 rats were given a single dose of 50 mg/kg i.p. streptozocin. Blood glucose levels were measured after one week and the rats whose blood glucose levels were over 300 mg/dl were included in the study. After 3 weeks diabetic rats were divided into four groups; diabetic control group, ALA group, diabetic exercise group, ALA exercise group. ALA groups received 100 mg/kg i.p. ALA for two weeks. The rats in the exercise group were exposed to exercise at 15th day. Intracardiac blood was taken under anesthesia and malondialdehyde (MDA) and nitric oxide (NO) levels and superoxide dismutase (SOD) and xanthine oxidase (XO) activities were determined in the blood samples. It was observed that ALA exercise group had lower level of MDA than diabetic exercise group. NO levels in the exercise groups supplemented with ALA were lower than in the diabetic groups performed exercise but not received this supplementation. We concluded that exercise induced lipid peroxidation is decreased by ALA supplementation and NO increase is attenuated by ALA supplementation in diabetic rats.</jats:p>

          <jats:p>Introduction: In this study, in order to shed light on the intraparenchymal pressure reduction mechanism of dexmedetomidine, the effect of two different doses of 80 and 320 µg kg-1 on the levels of vasoconstrictor Endothelin-1(ET-1) and vasodilator prostaglandin I2 (PGI2) in blood and cerebrospinal fluid (CSF) in transient global cerebral ischemia model in rabbits was examined. This study was conducted in the Selcuk University Experimental Medicine Research Center between June 2005 and January 2006 and approved by an ethical committee. All rabbits were randomly divided into four groups; six rabbits in each group. In Group 1, a craniotomy was performed and the ischemia was not created. In Group 2, only an ischemia-reperfusion was performed. During reperfusion, 80 µg kg-1 intravenous dexmedetomidine was administered to Group 3, and 320 µg kg -1 intravenous dexmedetomidine was administered to Group 4. After the application of craniotomy and cervical neck dissection of the rabbits in Groups 2, 3 and 4, the bilateral carotid arteries were clamped and an ischemia was performed for one hour. Then, the clamps were removed and reperfusion was performed for one hour. In addition, ıntraparenchymal pressure were recorded after craniotomy and reperfusion. Blood and CSF samples were collected from rabbits after craniotomy in Group 1 and reperfusion in Groups 2,3 and 4 to determine levels of ET-1 and PGI2. Intraparenchymal pressure values after the reperfusion in Group II and IV were significantly higher than Group I( p0.05). ET-1 levels of both blood and CSF were increased in the group performed ischemia and reperfusion and no treatment (Group II) and the group administered low dose dexmedetomidine (Group III) were similar with the sham group (p&gt;0.05). However, PGI2 levels of CSF were significantly decreased in the group administered low dose dexmedetomidine (p&lt; 0.05). Dexmedetomidine can reduce the intraparenchymal pressure by reducing the level of ET-1 in the transient global cerebral ischemia model.</jats:p>

        <jats:abstract>
          <jats:p>Although laparoscopic adjustable gastric banding has been shown to be a safe and effective weight loss procedure, band migration and erosion in to gastric wall has been reported in all series. Gastric band migration may lead to portocutaneous fistula. We report three cases. All of patients underwent laparoscopic adjustable gastric banding for morbid obesity and subsequently presented portocutaneous fistula associated with band migration.</jats:p>

          <title>The Effect Of Ketoprofen On Postoperative Hemorrhage Following Tonsillectomy</title>

          <jats:p>The objective of the study was to evaluate the effect of ketoprofen on after tonsillectomy in patıents. All charts of patıents, who underwent tonsillectomy with or without adenoidectomy, were reviewed. The age at the time of surgery ranged between 3 and 27 years (mean age = 10.0±3.02 years). Patıents were divided into two groups based on the drugs used for postoperative pain relief.Group I received paracetamol after surgery. Group II received Ketoprofen after surgery. A total of 40 patients received ketoprofen and 50 patients were given paracetamol. Posttonsillectomy hemorrhage occurred in one (2.5%) patıent,primary hemorrhage was noted in one patient and secondary hemorrhage occurred in zero patients. While 1 of 40 patıents (2.5%) who were given ketoprofen had postoperative hemorrhage,1 of 50 patients (2.0%) who were given paracetamol had hemorrhage There was no significant difference in hemorrhage rates between these two groups (p[0.05). Hemorrhage following tonsillectomy is rare and frequently occurs in the early postoperative period. There is no significant increased risk of hemorrhage after ketoprofen administration and it can be used safely for posttonsillectomy pain relief.</jats:p>

          <jats:p>A case report of congenital diaphragmatic hernia which was diagnosed in the 14 th week of pregnancy is presented. Case Presentation: 31 year old multigravida who had 14 th week of pregnancy underwent an ultrasound examination where the fetal abdominal organs were seen in the thoracic cavity. No other sonographical pathologies were determined. The family was counseled in detail and termination of pregnancy was decided. Genetical and pathological evaluation following the misoprostol induced abortion of the pregnancy did not reveal further abnormalities. The pathological examination described a posterior defect of the diaphragma, which leads to the herniation process, as the only positive pathological finding.</jats:p>

          <jats:p>Hydatid cyst is a parasitic infestation caused by Echinococcus granülosus. In this study we present a pulmonary hydatid cyst case that has radiological and bronchoscopic view like a malign mass. A 17 year-old male patient. He has cough, sputum and hemoptysis complaints. He has 10 kg weight loss in last two mounths. Thorax computed tomography showed a consolide lesion in the the right upper lobe. Fiberoptic bronchoscopy was performed in this case. There was an endobronchial hump lesion covered with necrotic tissue, in the right upper lobe anterior segment. The result of bronchoscopic biopsy was reported as ‘hydatid cyst’. We found a cystic lesion at the left liver lobe in abdominal ultsonography. Patient transferred for operation at thoracic surgery department. Hydatid cyst was rejected by operation. Hydatid cyst is usually located in the lower lobes of the lungs. Although cyst has classic radiologic, bronchoscopic findings, we should think hydatid cyst in different radiological and bronchoscopic findings too.</jats:p>

          <jats:p>Spontaneous achilles tendon ruptures are not common situations and may occur due to prolonged use of systemic corticosteroids. In this study, 43 years old male patient with Behçet’s disease who had spontaneous achilles tendon rupture during daily activities is presented and the literature about spontaneous major tendon ruptures (achilles, quadriseps) in patients who have systemic corticosteroids treatment was reviewed.</jats:p>

          <jats:p>Inguinal hernia operations are frequently performed at general surgery clinics. These procedures can be performed not only under general anesthesia but also with methods like spinal anesthesia, epidural anesthesia, and local anesthesia. Inguinal reparations can be successfully performed with local anesthesia especially on older patients, those with comorbidity and significant risk of anesthesia. An ample amount of studies in literature underline the fact that the results of hernia reparations performed under local anesthesia are no different than the results of hernia reparations done under general anesthesia. This report discusses the case of an 87-yearold male patient with comordid diseases like hypertension, diabetes mellitus, congestive heart failure, and chronic kidney failure as well as irreducible inguinal hernia who had to undergo emergency hernia reparation under local anesthesia. Although inguinal hernia reparation under local anesthesia has been comprehensively evaluated in literature, there is very limited information on the reparation of incarcerated inguinal hernias under local anesthesia. Consequently, we think that it is possible to use local anesthesia successfully to surgically treat incarcerated hernias with low risk of strangulation especially on senior and comorbid patients. However, we also believe that it would be of utmost significance to conduct a comparative study between emergency hernia operations performed under local anesthesia and general anesthesia in order to be able to recommend a safer solution on the subject.</jats:p>

        <jats:abstract>
          <jats:p>We report a patient with pancytopenia which is an unusual manifestation and a rare hematologic complication of acute brucellosis. A 29-year-old female patient was hospitalized with fever and pancytopenia. Brucella standard tube agglutination test was found to be positive and cultures from blood yielded growth of Brucella melitensis. The patient completely recovered by the sixth week following combined antibacterial treatment of doxycycline and streptomycine. Brucella should be considered as an uncommon cause of pancytopenia and should be treated immediately.</jats:p>

          <jats:p>Epidural hematomae are one of the most seen emergency neurosurgical pathologies. Acute epidural hematomae generally depends on the bleeding of the arteria meningia media and its branches and collection of the hematoma at the epidural space. Epidural hematomae rarely depend on the venous sinusal bleedings. The management is generally surgical decompression of the hematoma. In this report, we present a case which is not operated after the detection of epidural hematoma and managed conservatively.</jats:p>

          <jats:p>Diabetes mellitus (DM) absolute or relative deficiency of endogenous insulin or resulting from peripheral ineffectiveness of chronic hyperglycemia, carbohydrate, protein and fat metabolism disorder, capillary membrane changes, and accelerated atherosclerosis associated with chronic, progressive disease (1). The main cause of vision loss in patients with diabetic macular edema(4). In the treatment of macula edema, laser photocoagulation, VEGF inhibitors and vitrectomy is recommended. Choroid, neuroretina, photoreceptors preserving particularly selective RPE laser damage seems to be the appropriate treatment method (24). It was seen that Fundus was preserved µs pulse duration and consecutive irradiation damage, selective RPE with photoreceptors (26,27). Suffered damage to the planned study of selective RPE treatment in diabetic macular edema, Afyon Kocatepe University School of Medicine, Department of Ophthalmology, Retina Unit, between October 2005 and February 2007 were followed up with a diagnosis of DRP, and triggers as a result of the examination detected the presence of macular edema over the age of 18, 99 eyes of 84 patients separated into 2 groups and administered SRL and grid laser treatment followed 6 months. As a result, SRL, and grid laser photocoagulation was effective in the treatment of DMÖ. 84.9 % increase in SRL-treated patients and stabilization of visual acuities were determined. It was confirmed that in Grid laser group and 6-month follow-up and stabilization as a result of an increase in visual acuity was 89.9%.</jats:p>

          <jats:p>Psychiatric disorders and psychological stress has been implicated as a potential trigger in onset and exacerbation of dermatological diseases. Additionally, skin diseases have often been found to be associated with anxiety, depression and impaired quality of life. Therefore, it may be considered that there are a reciprocal influence between psychiatric status and skin diseases. Although significant majority of the cases begin before adulthood, limited data are available about relationship between skin disease and psychiatric morbidity in childhood and adolescence. In this article, psychodermatologic disorders in these age groups and our current scientific knowledge on these disorders will be discussed.</jats:p>
